Normally this is cuased during installation when you catch the polarization layer on the frame and pull it away from the lens medium. They are actually two lenses thermally fused together and seldom split.   However,  that said, it does depend on how they are installed.   Ensuring they are not forced over the rim, but slightly bent and placed into the rim is the best method.   Anyhow, I will have someone reach out to you and see what has happened.
